I appreciate the confidence you place in me. My goal is to achieve a seamless transition so patients feel like my office is an extension of yours. To help me accomplish this goal, please review this checklist when referring a patient:

• The more information I have about your patient the better! Please include tooth numbers/quadrants associated with your patient’s treatment needs. Also, please advise what you have done and what you want to do in the future so that I can reinforce your recommendations and be consistent in my treatment planning.
• Please email the most current x-rays (fmx/pan/PAs with date taken) for your patient.  The email address is info@drrobbins.net
• Your patients should be advised that they will be seen first for an examination.  Any procedure needed will occur at a separate appointment.
• My referral form is available Here. Please print the form and email it to me at info@drrobbins.net
• Please encourage your patients to continue care at your office while seeing me, even if it’s only for a restorative exam. This keeps them up-to-date in your office throughout their treatment here
