Your First Visit
Your medical and dental history will be reviewed with you during your initial visit. A comprehensive periodontal examination & oral cancer screening will be performed. Any necessary x-rays will be made to aid in diagnosing your periodontal condition. If you have recently had x-rays made at another office, please bring a copy to your appointment or call and let us know in advance so that we may request a copy from your dentist.
Next, we will develop a treatment plan for your case and review the financial costs associated with your care prior to initiating treatment. We will let you know how much your dental insurance is contributing to your treatment and help you maximize your benefits as much as possible.
Please remember to bring with you a list of any medications (prescription and over-the-counter) you are taking, along with the dosage of each medication to your first appointment.
Also, please bring your insurance card so that we can make a copy for your file.
We will contact your insurance prior to your appointment to obtain an outline of your benefits. Any information we receive prior to your appointment is strictly an estimate and can change based on the available maximum at the time of your treatment. We do our best to provide you with accurate coverage estimates based on information available to us.
Insurance reimbursement is a contract between you, your employer and your carrier, and you are responsible for the payment of your account within the limits of our financial policy.
